Miller, RPh<br \/>\nAPC President<\/p>\n<p>Last week, I addressed APC\u2019s opportunities \u2014 including membership, Tricare, FDA and technicians.<\/p>\n<p>The \u201cT\u201d in SWOT stands for <em>threats<\/em>.<\/p>\n<p>APC and pharmacy compounding have always faced threats, small and large. FDA overreach is an ongoing fight and probably the greatest threat we face; the agency just seems to become ever emboldened in its efforts to marginalize our profession.<\/p>\n<p>Just last month, in <a href=\"https:\/\/whizkid.com\/a4pc\/files\/2022-02_FDA-Response-to-Congress-cBHT.pdf\">a letter to members of Congress<\/a> who had written to the agency to urge caution in restricting compounded hormones, the agency stated:<\/p>\n<blockquote><p>\u201cFDA believes the results of NASEM\u2019s research provide important information that will increase public understanding regarding cBHRT products. <strong>When developing agency policies, FDA intends to consider the information in the NASEM report,<\/strong> along with information and comments received from members of the public, while taking into account patient access concerns.\u201d<\/p><\/blockquote>\n<p>FDA sticking with that debunked report isn\u2019t a surprise \u2014 but now there are rumors that it could move to put hormones on the \u201cdemonstrably difficult to compound\u201d list as early as this summer. We all know that hormones are <em>not<\/em> difficult to compound; three-quarters of APC members do it regularly, and they rely on compounded hormones as a clinical and business line. Without cBHT, many of us would cease to exist as compounders.<\/p>\n<p>(And don\u2019t forget that FDA also commissioned a NASEM report on compounded topical pain therapies. If it\u2019s successful with limiting cBHT, you can bet it will take similar action on those.)<\/p>\n<p>These threats doesn\u2019t apply only to humans. In its <a href=\"https:\/\/whizkid.com\/a4pc\/gfi256\">Guidance for Industry #256<\/a> the agency shows its intent to meddling in the practice of <em>veterinary<\/em> medicine \u2014 prohibiting compounding for animals from pure ingredients in most circumstances.<\/p>\n<p>These are threats, but they\u2019re also opportunities. And that\u2019s why I\u2019ve stressed increasing our membership, strengthening CompPAC, and working with agencies like FDA. HealNow does all the heavy lifting \u2014 pharmacies don\u2019t need to pay thousands of dollars to a third-party credentialing organization or spend months finding a payment processor that accepts online payments. <strong>With HealNow, your pharmacy can accept online payments in hours<\/strong>.<\/p>\n<p>Check out what going all-in with digital can do for your pharmacy, and how HealNow can make it happen: <a href=\"https:\/\/www.healnow.co\/\">HealNow.co<\/a>. <em>And when you sign up, be sure to tell them you\u2019re an APC member!<\/em><\/p>\n<\/div>\n<p><!-- ARTICLE BLURB ---------------------------------- --><\/p>\n<div class=\"story\">\n<h2>APC to USP: \u201cWhere\u2019s the science?\u201d<\/h2>\n<p>APC submitted formal comments this week regarding USP\u2019s proposed changes to beyond-use dates in General Chapters &lt;795&gt; and &lt;797&gt;.<\/p>\n<p><a href=\"https:\/\/whizkid.com\/a4pc\/files\/2022-03_APC-Letter-to-USPCEC-795-797.pdf\">The detailed, 92-page document<\/a>, largely formulated by APC\u2019s BUD Task Force, not only laid out in detail our comments on the language of the proposals, but provided the rationale behind each change we believe must be made.<\/p>\n<p>Most notably, we strongly urged USP to remove any references to batch size restrictions:<\/p>\n<div class=\"blockq\">\n<p><em>APC requests that USP remove any batch-size limits for CSPs and allow Category 3 CSPs to be assigned BUDs of up to one year, regardless of sterilization method or storage temperatures, as supported by data from stability-indicating studies, until USP can:<\/em><\/p>\n<ol>\n<li><em>Demonstrate that its proposed batch-size limitation and BUD restrictions are rooted in objective, replicable science and provide clear patient benefit; and<\/em><\/li>\n<li><em>Provide guidance as to stakeholder concerns over the effects of colder storage temperatures on compounded preparations, packaging, and patients themselves and the effects on patient access of cost-prohibitive stability studies<\/em><\/li>\n<\/ol>\n<\/div>\n<figure><img decoding=\"async\" src=\"https:\/\/whizkid.com\/a4pc\/files\/head-Tenille-Davis.png\" alt=\"\" align=\"right\" hspace=\"10\" \/>\u201cUSP has always had a reputation for relying on hard data and solid evidence, and we hope to see that continue,&#8221; said Tenille Davis of Civic Center Pharmacy in Scottsdale, Ariz., chair of the BUD Task Force. \u201cAll the concerns we\u2019ve raised result from the lack of evidence to support the proposed language of the chapter. I\u2019m confident the Compounding Expert Committee will appreciate the importance of continuing to base its standards and recommendations on solid science.\u201c<\/p>\n<p>Almost a year of work went into studying the USP proposal and developing APC\u2019s comments.
